The Wellness Program is a benefit designed to help employees better manage and improve their  health and wellbeing. This program identifies employee health risks; provide wellness coaching  and appropriate referrals to lifestyle and disease specific action plans, classes, seminars, medical  specialists, etc. in order to help employees take action to improve their health. Ultimately, this  program will reduce overall medical costs.

  • Employees participating in the Wellness Program will complete a Personal Health Assesment (PHA) and Biometric Screening annually.
  • Cotinine Testing for nicotine(via blood sample) – applies to all employees covered by the health plan
  • The PHA is a health questionnaire and a fasting blood draw, used to provide individuals with an evaluation of their health risks and quality of life.
  • The results of the PHA and Biometric Screening and recommendations will be provided to you. Your results cannot be viewed by anyone but you and the wellness coach and will not be released to anyone without your authorization.
  • Wellness coaching sessions may be required based on the results of the PHA and Biometric Screening.
